Endodontics
Root canal treatment — the procedure that keeps a tooth instead of extracting it.
When it is needed
When decay or injury reaches the dental pulp — the soft tissue containing the nerve and blood vessels inside the tooth. The pulp then becomes inflamed or dies, and infection spreads through the root tip into the surrounding bone.
Typical signs: prolonged pain that worsens at night, sensitivity to heat that does not settle once the stimulus is removed, pain on biting, swelling of the gum beside the tooth. Sometimes there are no symptoms at all and the change is found on an X-ray.
Endodontic treatment is the alternative to extraction. Your own tooth, even without its pulp, remains a better solution than any replacement because it preserves bone and bite.
How it is done
- X-ray and anaesthetic. The image shows the number and shape of the canals — which determines both the complexity and the cost.
- Opening the tooth and removing the inflamed or dead pulp.
- Measuring canal length, mechanical cleaning and irrigation. This is the decisive step: success depends on how thoroughly the canal is cleaned.
- Filling the canals and a check X-ray.
- Final restoration of the crown — a filling or, for badly damaged teeth, a crown.
How many visits
A straightforward single-canal tooth is often completed in one visit. Multi-canal teeth and teeth with marked infection need two or more, because medication is placed in the canal between visits.
The number of canals is not a matter of estimation but visible on the X-ray, which is why the price differs for single-, two- and three-canal teeth.
Apicoectomy
If infection at the root tip does not resolve even after properly performed canal treatment, an apicoectomy is carried out — surgical removal of the root tip together with the focus of infection, preserving the rest of the tooth.
It is a second-line procedure: the conventional route through the canal is tried first, and apicoectomy follows when that route gives no result or is not passable.
